Shanahan sustained multiple injuries, including a concussion, broken hand, nose, ribs, and stitches above his eye, in the July 14 crash.

He explained that he took his eyes off the road for a moment to retrieve a dropped phone, causing his Tesla to veer into the other driver's SUV. The accident occurred at approximately 6:12 p.m. near Alma Street and Hamilton Avenue in Palo Alto, California. Shanahan expressed distress that the initial report wrongly blamed the other driver.

Jeremy Jones, the 49ers' vice president of security, suggested the discrepancy was likely a clerical error during data entry into the California crash reporting system. This error has been flagged and is being corrected to reflect Shanahan's admission of fault. While recovering, Shanahan has been able to attend 49ers practices on a limited basis, though he notes fatigue from his concussion is a significant challenge.
