An inquest into the death of former Sheffield United women's player Maddy Cusack has revealed disturbing allegations about her relationship with coach Jonathan Morgan. The 27-year-old was found unconscious at her family home in Horsley, Derbyshire on September 20, 2023 and died the same day.

Grace Riglar, Cusack's girlfriend and teammate, testified that Morgan had called Cusack a "psycho" during a match while at Leicester City, and made comments about her weight and relationship after joining Sheffield United. Riglar said Cusack became anxious when Morgan arrived, having previously worked with him.

The inquest heard that Cusack's playing time dramatically decreased under Morgan's management, going from a guaranteed starter to frequently benched. Riglar described this as Cusack viewing it as a "personal attack" and believing Morgan was deliberately playing mind games. Morgan's practice of calling Riglar "Mrs Cusack" in front of players made the couple uncomfortable.

Cusack's mental health deteriorated significantly, with Riglar describing her as becoming "paranoid" and changing her eating habits by cutting carbohydrates and skipping meals. She had been prescribed anti-anxiety medication and obtained a sick note before her death. The player had expressed desires to leave football entirely, telling Riglar she wanted to move to Dubai to become a flight attendant.
