Medicare's new coverage pathway for weight-loss drugs like Wegovy and Zepbound represents a milestone for patients, but offers limited upside for manufacturers. The Bridge program provides access to eligible seniors for a $50 monthly copay, ending years of exclusion for many older Americans seeking GLP-1 treatments.

Federal law previously barred Medicare from covering weight-control medications, leaving millions of seniors unable to afford these expensive drugs. Eli Lilly and Novo Nordisk viewed Medicare as the holy grail market for obesity treatments, representing the nation's largest drug demand pool. However, the actual addressable market is smaller than headlines suggest.

While up to 20 million U.S. seniors are overweight or obese, the Bridge program's eligibility rules are stricter than FDA approvals. The temporary program requires higher weight thresholds or serious related conditions, and excludes anyone already receiving GLP-1 coverage through standard Medicare for diabetes or sleep apnea.

Investors expecting immediate revenue windfalls from Medicare coverage will likely be disappointed. The narrow eligibility criteria and temporary nature of the program mean any sales boost will be gradual and uncertain, despite the symbolic importance of finally cracking the Medicare market.
