Adam Johnson, a 29‑year‑old forward for the Nottingham Panthers, collapsed after a neck collision with Sheffield Steelers’ Matt Petgrave in an Elite League match at Sheffield Arena in October 2023. Paramedics described the scene as “chaotic,” and Johnson was taken to hospital where he later died from a fatal neck injury. The incident shocked the community.

Sheffield senior coroner Tanyka Rawden, in a pre‑inquest review, questioned how the Yorkshire Ambulance Service accessed the ice, noting a paramedic had to climb over barriers because no one could guide them onto the rink. She also raised concerns about the availability of blood and the speed of medical care provided for patients immediately after.

The inquest will focus on the cause of Johnson’s neck injury, the arena’s medical provision, and the timeliness of care. A prevention report already warned Ice Hockey UK and the English Ice Hockey Association that deaths could recur if neck guards remain optional, giving bodies 56 days to respond and follow up within the next.

Petgrave, who was in Canada when the hearing was held, remains uncharged after the Crown Prosecution Service declined to prosecute. Fans and clubs worldwide paid tribute, while Rawden said the inquest may not occur this year. The tragedy underscores the need for stricter safety protocols in a sport where neck injuries are increasingly fatal to.
