Adults need 560-610 minutes of moderate to vigorous exercise weekly to achieve substantial cardiovascular protection, according to a British Journal of Sports Medicine study. This represents 3-4 times higher than current public health recommendations of 150 minutes per week. The research followed 17,088 UK Biobank participants for 7.8 years, tracking exercise levels via wrist-worn devices and measuring cardiorespiratory fitness.

Participants meeting only the 150-minute guideline saw just 8-9% risk reduction for heart attacks and stroke. In contrast, those reaching 560-610 minutes weekly achieved over 30% risk reduction. Less fit individuals required approximately 30-50 additional minutes per week compared to highly fit participants to gain equivalent benefits, highlighting challenges for deconditioned populations.

The study measured VO2 max—the maximum oxygen consumption during intense exercise—as a key indicator of cardiorespiratory fitness. Researchers found that low fitness strongly predicts cardiovascular risk, with 1,233 cardiovascular events occurring during the follow-up period. Current guidelines provide a basic safety margin, but optimal protection requires substantially higher activity volumes tailored to individual fitness levels.

These findings suggest public health messaging may need updating to differentiate between minimal protective exercise and optimal cardiovascular risk reduction. Personalized fitness targets based on individual baseline fitness could help bridge the gap between current recommendations and what research shows actually works for heart health.
