Pentagon investigators found that flawed intelligence, outdated imagery, and overreliance on AI contributed to a missile strike that killed 123 children in Minab. Two Tomahawk missiles hit the Shajarah Tayyebeh Elementary School on Feb. 28, killing over 150 people. It is the deadliest American military targeting error of the 21st century.

The strike resulted from an accumulation of smaller failures, not a single catastrophic decision. The Trump administration's demand for an overwhelming aerial assault compressed targeting time, while gaps in information, cuts in civilian-protection personnel, and overreliance on AI set the stage for the error.

Admiral Brad Cooper ordered the investigation. President Donald Trump has suggested Tehran was at fault. The UN said there are reasonable grounds to conclude the strike amounted to a war crime. The full Pentagon report has not been released.
