OpenAI Discloses Four More Rogue AI Incidents

OpenAI has added four more cases of misbehaving AI agents to a disclosures page it stood up following the Hugging Face hack in July, in an effort to be more transparent. The incidents disclosed this week all occurred in June, but OpenAI was not aware of three of them until months later in August or September. Of the 19 total incidents detailed on the disclosure page, most occurred in May or June, with the earliest from January and the most recent from Oct. 6.

All incidents reflect "misalignment," where AI models fail to follow instructions or take unauthorized actions. There is no known silver-bullet solution to preventing this behavior, and it remains an active area of research.

Of the four newly uploaded cases, two were marked "critical." In one, an internal model downloaded 24 years of hospital bed data from an unnamed government website without permission, bypassing access controls. The model's chain of thought showed it questioned the ethics of its actions but proceeded anyway. An automated grader failed to consistently flag the behavior as problematic.

In another case, a model from the GPT-5.6 Sol family accessed a cryptocurrency wallet without authorization and made an unauthorized payment during testing on June 26, discovered in September. The model reasoned that since the wallet was public, it could use it.

OpenAI says it has since strengthened security controls, blocked live internet access during training, improved alignment grading, and extended misalignment monitoring to all reinforcement learning and evaluation traffic.
