Biograph Study: 42.9% Show Hidden Coronary Inflammation

Biograph, a preventive health and diagnostics clinic, has completed its first U.S. study using Caristo Diagnostics' AI-powered Ca Ri-Heart technology to detect coronary inflammation in asymptomatic, low-risk individuals. The study, available to clinically eligible Biograph Black tier members, combined high-resolution coronary computed tomography angiography (CCTA) imaging and cardiometabolic profiling with Caristo's Ca Ri-Heart and Ca Ri-Plaque technologies.

The findings show that coronary inflammation is common. 42.9% of participants had elevated inflammation in at least one major heart artery, and 17.9% had very high inflammation. Notably, about 47% of participants with no detectable coronary plaque still showed elevated perivascular inflammation, suggesting a limitation in standard assessment that relies on plaque and stenosis alone.

Importantly, routine bloodwork and clinical tools, including cholesterol markers (Apo B, non-HDL-C), blood sugar (Hb A1c), systemic inflammation (hs-CRP), and the AHA PREVENT-ASCVD risk calculator, showed no correlation with coronary inflammation and failed to identify which patients had inflamed arteries. Researchers said the results reinforce that isolated metrics do not tell the full story.

The study's authors described coronary heart disease as a likely late consequence of an active inflammatory process in the vessel wall. They suggested the findings may identify an earlier phase of disease, creating a window for proactive intervention. They also stressed that more research is needed to validate the results and establish the prognostic value of treating coronary perivascular inflammation.
