Retailers are grappling with a wave of returns as shoppers on weight-loss medications rapidly shed pounds. The trend is hitting apparel companies particularly hard, with customers buying multiple sizes of the same garment and returning ill-fitting items. This shift reflects the broader adoption of GLP-1 drugs like Ozempic and Wegovy, which have transformed consumer shopping patterns nationwide.

Farnam Elyasof, founder of online budget suit retailer FlexSuits, has seen a 50% increase in returns over the past year. When customers order the same suit in two or three sizes, Elyasof treats it as a "red flag" and probes whether they are actively losing weight. He often advises clients to delay purchases until closer to their events, though returns continue flowing in regardless.

Data from returns-management firm Narvar shows the share of apparel exchanges where shoppers sized down climbed to 14.6% in 2025, up across each of the past three years. The firm analyzed 38 retailers to reach this conclusion. This sizing shift represents a fundamental change in how Americans shop for clothes.

For retailers, the trend creates margin pressure as they absorb return shipping costs and inventory losses. Companies are adapting their return policies and sizing guidance, but the underlying driver—millions of Americans taking weight-loss medications—shows no signs of slowing down.
