Legion Health, a Y Combinator-backed startup from the S21 batch, is hiring a founding engineer to build out its AI-native operations infrastructure for mental health care. The San Francisco-based company, which runs its own psychiatric practice, is looking for someone to architect the backend systems that manage everything from scheduling and billing to risk detection and documentation. Co-founder and CTO Daniel Wilson, a Princeton grad and former Microsoft engineer, is leading the search.

He wants a builder who can own core systems end-to-end, designing event-driven backends and reliable LLM agents that function as actual teammates. The role involves working with Node.js, TypeScript, Supabase, and AWS to create a system that coordinates human clinicians and AI agents. Legion claims its agent infrastructure already supports over 2,000 patients with just one human support lead.

The company is offering a base salary between $130,000 and $190,000, plus meaningful early equity. This move reflects a broader trend of healthcare startups attempting to use AI to fix the sector's notoriously inefficient administrative backend.
