The continued adoption of Retrieval-Augmented Generation exposes latent failure modes where retrieval accuracy does not translate to factual output veracity, even when document scores are perfect. Researchers demonstrated this issue using a controlled 220 MB local experiment, showing that retrieving the correct data is insufficient if downstream synthesis logic remains flawed.
