Bacteria Evolves in UTI, Invades Brain

Researchers have documented a concerning case where bacteria from a urinary tract infection (UTI) evolved within a patient over two years, eventually spreading to her brain. Genome sequencing revealed that the bacteria in urine samples diverged from a common ancestor one to two years prior to the patient's brain abscess. Subsequently, a hypervirulent strain disseminated from the urinary tract to the brain.

While bacterial adaptation within a host is known, this case demonstrates a phenomenon termed 'heterovirulence.' Unlike 'heteroresistance,' where antibiotic resistance comes with a fitness cost, here a subpopulation of bacteria gained an advantage for dissemination and virulence, despite a disadvantage in the urinary tract. All three bacterial isolates from the patient were susceptible to antibiotics, and she was successfully treated.

This instance highlights *Klebsiella pneumoniae*'s capacity to spread from a urinary reservoir to distant sites within a patient and underscores the potential for such emergent virulence. The researchers emphasized that this case showcases the bacteria's ability to disseminate and the emergence of new pathogenic traits.
