The fallout from Erling Haaland's goal in Manchester City's 1-0 win over Manchester United at Old Trafford continues. VAR Matt Donohue and assistant VAR Blake Antrobus awarded the goal in error, failing to spot Enzo Fernández offside, as Pro Ref admitted. The audio released Tuesday shows the mistake.

This isn't isolated. Luis Díaz's goal for Liverpool against Tottenham in October 2023 was wrongly disallowed, prompting an apology for a "significant human error" by VAR Darren England and assistant Dan Cook. Eight months earlier, Arsenal got an apology after Lee Mason's error led to Ivan Toney scoring for Brentford.

Soccer should follow the NFL, tennis, and cricket by introducing a challenge system. In the NFL, coaches throw a red flag; in cricket, captains have 15 seconds. Penalties exist for failed challenges. If Michael Carrick could have challenged Fernández's offside, the error would have been erased. As VAR remains imperfect, a challenge system is needed.
