### NTSB Updates B-767 Runway Accident Investigation in Miami

WASHINGTON (Sept. 9, 2026) — The National Transportation Safety Board is providing the following update on its investigation into the Sept. 6 runway excursion accident involving 21 Air Flight 7598, a Boeing 767-33A cargo airplane, at Miami International Airport. The following information from the cockpit voice recorder (CVR) and flight data recorder (FDR) is preliminary and subject to change. The NTSB recovered the CVR and FDR from the accident site and transported them to the NTSB’s Vehicle Recorder Laboratory in Washington, DC, where they were successfully read out. The aircraft was equipped with an Acron Aviation (formerly L3Harris) model FA2100 CVR and an Ontic (formerly Honeywell) model SSFDR FDR.

The CVR contained more than 2 hours of good quality audio across four channels. The FDR contained approximately 54 hours of data and recorded more than 400 parameters. The NTSB will convene a CVR group in the near future at NTSB headquarters in Washington, DC, to review the accident flight recording and produce a written transcript.

A summary of select events captured on the CVR during the final few minutes of the recording is provided below. All times are referenced relative to the end of the CVR recording. At 01:42, a pilot called for flaps to be set to 20 degrees, and the other pilot cautioned they were too fast. The other pilot made additional comments regarding the aircraft’s excessive speed throughout the remainder of the recording.

At 00:58, electronic “sink rate sink rate” callout occurred. At 00:48, electronic “too low terrain” callout was annunciated four times. At 00:31, sounds similar to the aircraft touching down were heard. At 00:15, a pilot called for a go-around. At 00:10, sounds similar to the aircraft departing the paved surface were heard. The CVR recording ended at 00:00.
